D.A.D.S of STL is a fast-growing nonprofit organization that is committed to the encouragement, education and empowerment of Fathers. We work with dads, to ensure lasting impact through teaching advocacy, leadership, guidance, social-emotional intelligence, assertive self-expression and healthy relationships, giving them the skills and confidence to thrive as fathers.
Virtual Platform Release
I give permission for my child/children to participate in virtual programming via virtual platform, such as ZOOM, Microsoft Teams, WebEx or Lifesize. Students are expected to show up prepared, ready to engage and are not to cause any disruptions in the virtual classrooms or breakout sessions. Students are not allowed to post any derogatory statements in chat boxes. Students are not allowed to share the meeting link with students who are no enrolled in the program. Failure to comply will result in termination from the program. If students are absent for day school, they cannot participate in the afterschool program for that day.
Meeting links will be sent to the parental email address on this application. It is recommended that you create an email address for your child and forward the link to them.
Photo/Video Release
I, authorize D.A.D.S of STL Mentorship Program and its agents and employees the irrevocable and unrestricted right to reproduce the photographs and/or video images taken of me, or members of my family, for the purpose of publication, promotion, illustration, advertising, or trade, in any manner or in any medium. I hereby release D.A.D.S of STL Mentorship Program and its legal representatives for all claims and liability relating to said images or video. Furthermore, I grant permission to use my statements that were given during an interview or guest lecture, with or without my name, for the purpose of advertising and publicity without restriction. I waive my right to any compensation.
